Get to Know You
This form is used to help get to know the students at the beginning of the year by asking some simple questions.
Parent / Guardian Information

Tools:
What information do you want to collect from parents / guardians that might not be in your SIS?
Behavioral Record

Tools:
Keeping an accurate track of student behavior is easily managed with this form.
Making Predictions

Tools:
Students are asked to read five situations and then make a prediction based upon the information provided. This form uses an add-on that will automatically grade the assignment based on a key provided by the instructor. A variety of advanced options are available for feedback.
Fact or Opinion?

Tools:
Given a topic, students are to watch a video and read several statements to decide if each is a fact or an opinion.
Self-Evaluation Rubric

Tools:
After completing a class project, students use this form to self-evaluate their performance on the task.
Reading Comprehension

Tools:
Students are to read a passage and then answer questions based on the text to determine their comprehension. The form is automatically graded for the teacher.
Daily Question

Tools:
This Google form is used as a daily question that students can answer as a part of their morning routine.

My Maps Example - Places to Visit

Tools:
Using data sent from a form, a Google Map is dynamically generated with the information. In this example, students are asked to choose one location the would like to visit anywhere in the world and explain why.
